Military Medal 'For conspicuous gallantry and devotion to duty during the operations in front of Crevecouer on 8th October, 1918. Rifleman Forsyth took control of his section and when his platoon was held up by machine-gun fire he attacked from the flank and forced the enemy to retire over the open. His section immediately shot down several of them and also captured many prisoners. This man's work in action has been consistently very gallant, and he has shown plenty of initiative and courage.'
