Military Medal 'For conspicuous gallantry and devotion to duty. On 12th October 1917, during an attack on Passchendaele Ridge, Private Lark repeatedly brought in wounded under heavy shell and machine gun fire. He continued going while suffering from the effects of mustard gas.'
