Military Medal 'Operation: British front in the vicity of Foret de Mormal - 5th November 1918. For consicuous gallantry and devotion to duty. During the above operations Corporal Cusack was in command of a Lewis Gun section, and when his Company was held up by machine gun fire he pushed his gun forward on the flank, silencing the enemy machine guns. He then rushed and captured two machine guns. By his own bravery and disregard for his personal safety he set a magnificent example to all ranks.'
