Admitted to No 4 NZ Field Ambulance, 17 September 1917 (scabies); transferred same day to No 4 Stationary Hospital, Arques; discharged to unit, 23 September 1917; rejoined 3rd Bn, in the field, 23 September 1917.

Killed in action, Belgium, 19 October 1917.
