Military Medal 'Operations: British front in the vicinity of the Foret de Mormal 5th November 1918. For conspicuous gallantry and devotion to duty. During the above operations, the Company to which Sergeant Smith belongs attacked. On reaching the edge of the
wood, near the final objective, he led his platoon in a charge against three enemy machine guns with great bravery. This party killed the whole of two gun crews and captured the third. Later, he went on to the last possible position and consolidated. His initiative, daring and coolness inspired his platoon with great confidence and with the true spirit of the offensive, and throughout the day he rendered very valuable service to his Company.'
