Military Medal 'Action before Polderhoek Chateau east of Ypres, 3rd December 1917, and following days. For conspicuous gallantry and devotion to duty. On 3rd December 1917,
during an attack on Polderhoek Chateau, Private H.R. Slade worked under shell fire at his cooking continuously for twenty four hours, and through his resourcefulness, courage and energy, wounded were amply served with hot drinks, and 300 bearers were provided with hot food during their work.'
