Military Medal 'Operations in front of Crevecoeurt (Sheet 57B G.35.d.) on October 1st 1918. For conspicuous gallantry and devotion to duty. Sergeant Perry was in charge of the forward Bearer Relay Post. Throughout the day he was under constant heavy shell fire and he passed backwards and forwards between the regimental aid posts and B.R.P. arranging for and superintending the evacuation of wounded.'
