Military Medal 'For conspicuous gallantry and devotion to duty during the operations round Crevecoeur from 29th September 1918 to 3rd October 1918. Private Jacques is a Brigade Observer and as such has been constantly employed in the front line during the whole operations under the most intense machine-gun and artillery fire. His work in locating he positions of troops on the right flank of the Brigade during the taking of Bon Avis Ridge was magnificent. In front of Crevecoeur he worked his way round all our front line posts in spite of the heaviest machine-gun fire and the information he brought back was of the greatest value to the Brigade.'
